Position Summary:
As a Behavioral Health Nurse (BHN) in home health, you will play a vital role in delivering high-quality, patient-centered care for individuals with behavioral health needs. You’ll collaborate with physicians, specialists, and healthcare partners to ensure holistic, effective care that promotes patient safety and well-being.
Key Responsibilities:
Conduct initial and ongoing comprehensive assessments, including physical and behavioral health evaluations.
Develop and modify individualized care plans to address patient needs and ensure optimal outcomes.
Administer prescribed medications and treatments; provide therapeutic and rehabilitative interventions.
Educate patients and families on health management, preventive care, and recovery strategies.
Collaborate with healthcare professionals and community resources to coordinate care seamlessly.
Maintain accurate and timely clinical records and documentation, ensuring compliance with healthcare regulations.
Identify discharge planning needs and coordinate follow-ups to ensure smooth transitions of care.
Participate in on-call rotations and ensure patients receive timely support.
Coordinate necessary medical equipment and services for patient comfort and recovery.
Qualifications:
Graduate of an accredited nursing program with an active RN license.
At least one year of recent psychiatric RN experience (within the past three years).
Active state RN license in California.
Valid driver’s license and auto insurance.
Bachelor’s degree and one year of home health care experience preferred.
Strong observation, communication, and problem-solving skills.
Experience with Homecare Homebase (EMR) preferred.
Compensation:
$50 - $60 per hour, based on experience.
