Position Summary:
As a Behavioral Health Nurse (BHN) in home health, you will play a vital role in delivering high-quality, patient-centered care for individuals with behavioral health needs. You’ll collaborate with physicians, specialists, and healthcare partners to ensure holistic, effective care that promotes patient safety and well-being.

Key Responsibilities:

• Conduct initial and ongoing comprehensive assessments, including physical and behavioral health evaluations.

• Develop and modify individualized care plans to address patient needs and ensure optimal outcomes.

• Administer prescribed medications and treatments; provide therapeutic and rehabilitative interventions.

• Educate patients and families on health management, preventive care, and recovery strategies.

• Collaborate with healthcare professionals and community resources to coordinate care seamlessly.

• Maintain accurate and timely clinical records and documentation, ensuring compliance with healthcare regulations.

• Identify discharge planning needs and coordinate follow-ups to ensure smooth transitions of care.

• Participate in on-call rotations and ensure patients receive timely support.

• Coordinate necessary medical equipment and services for patient comfort and recovery.

Qualifications:

• Graduate of an accredited nursing program with an active RN license.

• At least one year of recent psychiatric RN experience (within the past three years).

• Active state RN license in California.

• Valid driver’s license and auto insurance.

• Bachelor’s degree and one year of home health care experience preferred.

• Strong observation, communication, and problem-solving skills.

• Experience with Homecare Homebase (EMR) preferred.

Compensation:
$50 - $60 per hour, based on experience.
