    Position Summary: Primary Healthcare Centers is seeking a Licensed Behavioral Health Specialist (LPC, LMFT, or LCSW) to join our integrated care team. This role plays a vital part in improving patient wellness by providing counseling, crisis intervention, and brief behavioral consultations in a primary care setting. The ideal candidate is patient-centered, collaborative, and passionate about making a difference in the lives of others.

    Key Responsibilities:

    Deliver individual therapy, behavioral consultations, and psychoeducation using evidence-based practices.

    Collaborate with providers and clinical staff to create and support patient-centered care plans.

    Assess and address mental, emotional, and social needs, including crisis intervention when necessary.

    Provide referrals, coordinate care, and support patients with chronic conditions.

    Maintain accurate and timely documentation in the EHR.

    Participate in outreach events and community engagement activities.

    Support continuous improvement of integrated behavioral health practices.

    Provide leadership and mentorship to other behavioral health staff if serving in a lead role.

    Qualifications:

    Master’s degree in a human services field (required).

    Active LPC, LMFT, or LCSW licensure in the state of Georgia (required).

    Minimum of 5 years' experience in behavioral health; experience in primary care preferred.

    Proficient in motivational interviewing and evidence-based therapeutic interventions.

    Strong documentation, communication, and collaboration skills.

    Ability to assess and respond to crisis situations effectively.

    Experience with EHR systems and Microsoft Office Suite.

    Valid driver’s license required.

    Additional Information:

    Patient populations served may include adults, geriatrics, children, and adolescents.

    Some remote work may be permitted.

    Position is subject to all current CMS vaccine mandates.
