Responsibilities

• Conduct comprehensive assessments to identify clients' needs, strengths, and goals
• Develop individualized care plans and coordinate services to meet clients' social, emotional, and practical needs.
• Provide counseling and crisis intervention to individuals and families facing various challenges, such as domestic violence, substance abuse, mental health issues, and homelessness.
• Collaborate with community partners and agencies to facilitate access to resources, including housing assistance, healthcare, employment, and educational opportunities.
• Advocate for clients' rights and needs by participating in case conferences, court hearings, and interdisciplinary team meetings.
• Maintain accurate and up-to-date documentation of client interactions, progress, and interventions, ensuring compliance with legal and regulatory requirements.

Qualifications

• Bachelor’s or master’s degree in social work (BSW/MSW)
• MUST have at least 1-3 years of nursing home or Long-term care experience.
• Strong knowledge of social work principles, theories, and best practices
• Excellent communication and interpersonal skills, with the ability to establish rapport and build trusting relationships with clients and multidisciplinary teams.
• Proficient in conducting assessments, developing care plans, and coordinating services.
• Demonstrated understanding of cultural competence and experience working with diverse populations
• Ability to work independently, as well as collaboratively within a team environment.
• Strong problem-solving and crisis intervention skills
• Compassion, empathy, and a genuine desire to make a difference in the lives of others.
