Job description
o Case aide will assist case managers or RN/PHN with case coordination. Case aide to assist with with scheduling appointments for staff, uploading documents into case works, to assist with mail, service agreements, and office coverage.
o Case Aide will assist with coordination with counties on forms, reassessments, and communication. Database Management: Create new files, placing paperwork in applicable folders, and maintenance of database information.
o Case Aide will assist with all mail: Mail Distribution: Distribute mail items, scanning, emailing, and saving in database.
o Case Aide will assist with auditing and tracking cases.
o Case Aide will assist with follow-up for missing documentation with providers, PHN/RN and/or case managers.
o Case Aide will assist with all clerical tasks.
o Case aide will assist with uploading and downloading required forms.
o Case Aide to assist with all initial contacts for cases: all initial phone contact, mailing out paperwork, and coordination with county on new PCA referrals.
o Serve as back up staff for other case aides. Case aide may provide back up for Case managers.
o Required for staff to be in the office 1-2 day per week. Staff can be remote for other days, and come to office as needed for meetings, paperwork, and coverage.
o Other Duties as assigned.
o Applicants must successfully clear local and/or stated background checks.
o Preferred Qualifications: One to two years’ experience in service coordination or case management, and clerical work is preferred, but not required.
o Applicants will be required to have internet at home.
Job Type: Full Time
Salary: $17-21 p/ hour.
Experience and education:
Case Management: 1 year (Preferred)
High School diploma or Higher
Experience in Clerical or Administrative Work
Preferred- speaks a second language.

Schedule:
Monday to Friday
Schedule will be determined with hiring supervisor.
Work Remotely:
All case aides are equipped to work remotely.
Case Aide will be required to meet with supervisor weekly via teleconference or in person during probation.
Case Aides will be required to attend monthly team meetings in person.
Case aide will provide coverage for other case aides as needed.
