    DIRECT CARE III / TEAM SUPERVISOR

    We're looking for TEAM SUPERVISOR for our ACCS Outreach team who will assist persons in need of psychiatric services acquire the skills necessary to live independently in the community. S/he works within the Agency’s team model, establishing open communication, support, and accountability among co-workers.

    ESSENTIAL JOB FUNCTIONS:

    AGENCY SERVICES-necessary to meet overall agency goals and provide support to   management and program staff.   75-90%

    • Provides direct support and supervision to Direct Care I and II staff.
    • Provides crisis intervention & adheres to program policy and procedure manual.
    • Participates in team approach, sharing responsibility for Persons’ services.
    • Notifies senior supervisory staff immediately in the event of a crisis, suspected illegal activity, or any behavior constituting an incident in the program.
    • Assists Persons with activities of daily living e.g. Personal hygiene, meal preparation, housekeeping, laundry, money management, etc.
    • Transports Persons in matters related to service delivery, e.g. medical appointments, shopping, job interviews, vocational program, banking, laundry.
    • Administer medication accordance with MAP policies and procedures.
    • Interacts with Persons in order to assess needs, provide information, discuss problems, identify patterns and stressors, and encourage growth and development.
    • Visits Persons.

    ADMINISTRATIVE/RECORD KEEPING RESPONSIBILITIES – necessary for maximizing department efficiency:5-10%

    • Maintains Persons’ records in accordance with standard format and timetables to include assessment and progress reports/service notes, incident reports, money disbursements, and other specialized documentation as required.
    • Attends bi-weekly team meeting, clinical meeting & supervisory meeting with Program Manager.

    COLLATERAL/ENVIRONMENTAL SERVICES – necessary to respond to external environment which influences the department.  5-15%

    • Able to react to change productively and to handle other tasks assigned due to changing environments.
    • Works within processes and procedures of referral source.
    • Liaisons with other service-providing agencies.
    • Strengthens working relationships with significant resources in Persons’ network as specified by senior supervisory staff.

    PROFESSIONAL GROWTH AND DEVELOPMENT – necessary to develop to full potential professionally: 5%

    • Seeks and uses training opportunities external to the Agency.
    • Attends agency-required trainings.
    • Engages in self-evaluation to develop professional goals.
    • participates in relevant Agency and/or community task forces.

    Required Skills

    • Knowledge of Persons’ behavioral dynamics.
    • Ability to respond effectively to crisis.
    • Good oral and written communication skills.
    • Ability to structure and organize responsibilities with minimal supervision.
    • Physical and mental demands are located at end of job description.
    • Ability to pass certification for CPR, First AID and MAP.
    • Valid driver’s license; reliable transportation.
    • Ability to provide goal-oriented services, to counsel, and the ability to safely transport Person.
    • Master’s degree and/or significant experience working with the population.
    • Supervisory experience.

    MINIMUM QUALIFICATIONS

    • Master’s degree and/or significant experience working with the population.
    • Supervisory experience.
    • Bilingual [English + Spanish] preferred.
