    CASE MANAGER

    MULTIPLE LOCATIONS IN GREATER SPRINGFIELD

    We're looking for a Case Manager to work within the Co-Occurring Enhanced Residential Rehabilitation Services (RRS)’s team model; assist clients to obtain needed services by providing information, referral coordination and follow-up; develop and connect with needed resources through information and referral, coordinate MAT services, meet weekly with clients assigned to develop a comprehensive assessment and Individualized Treatment Plan.  Facilitation of Evidence-Based Protocols, documentation of services and weekly progress.  Develop case conference information to present to the clinical team. Provide leadership in the absence of supervisory staff to direct care staff.  To assist the client with discharge and aftercare planning. To provide crisis intervention; attend trainings and staff meetings and maintain client records as per licensing standards.

    DUTIES INCLUDE:

      • Provide oversight for the designated SUD program under the direction of the Program Director.
        • To be capable of assuming enhanced responsibilities for providing advocacy services to clients.
        • To complete adult comprehensive assessment, treatment plans, group progress notes, weekly progress notes, discharge summaries, and aftercare planning.
        • To conduct intake interviews, administer and complete the ESM/EIM forms on Virtual Gateway and adhere to funding source requirements.
        • To facilitate treatment groups; meet weekly with assigned clients, and provide advocacy services whenever appropriate for each client.
        • To develop strong, supportive relationships with the clients, providing for their safety and care.
        • To maintain client records as per licensing standards.
        • To arrange for or transport clients to appointments in the absence of other means of transportation.
        • To develop case conference documents for clinical supervision group/meeting.
        • To collaborate with other departments in settings where care must be coordinated between medical, MAT, and psychiatric and behavioral needs.
        • To work in collaboration with primary care providers, employees, and outside consultants, as necessary.
        • To participate as a member of the multi-disciplinary team meetings, staff meetings, and trainings.

    Required Skills Required Experience

    QUALIFICATIONS

    • Associates Degree + 2 years’ experience— working with SUD and moderate to severe MH disorders required.
    • Bachelor’s Degree + 2 years’ experience—working with SUD and moderate to severe MH disorders preferred.
    • Experience providing SUD treatment services, including group facilitation.
    • Valid driver’s license, auto insurance, ability to assume travel assignments related to work.
