    Bi-Lingual Family Support Case Manager

    Full-Time Location: Greenfield, MA with travel throughout Hampshire and Franklin Counties as needed. Salary: $ 20.32/hour

    ServiceNet, a leading Human Services agency, is seeking compassionate, dedicated, and enthusiastic individuals to join our team. In Family Empowerment, our Family Support Case Managers are a key resource to families who have a member with a disability. You will provide training, needed information, family support contacts and referrals to other agencies and supports. On a wider level, you will help educate the community about disabilities and/or autism and how to help better support families, as well as facilitating social and recreational activities for people of all ages. You will help families get the best results from funds available by administration of stipends and all documentation.

    Key Responsibilities:

    Providing individualized support to families.

    Help develop support plans/goals when needed.

    Help families to navigate resources and services available to them.

    Maintain contact and ongoing support as needed.

    Connect families with other families for support.

    Promote community integration.

    Help maintain and expand an extensive community-based network of support & informational resources.

    Develop center-based and community-based activities, groups, training and events.

    Qualifications:

    High School Diploma / GED required.

    Bi-Lingual using English and Spanish.

    Over 1-year direct experience in similar human services agency case work with families or individuals with disabilities.

    Broad knowledge of disability support services.

    Valid driver's license and reliable transportation/vehicle required.
