We are seeking a dedicated and experienced Medical Case Manager to join our growing team. The ideal candidate will possess a strong background in medical case management, with a keen ability to advocate and coordinate their care with our provider effectively. Fluency in Spanish is a must for this role. Responsibilities: • Assist the Provider in keeping track of where patients are in their treatment. • Help coordinate medical appointments, treatments, and other healthcare services. • Communicate effectively with providers and staff on where the patient is in their care. • Maintain accurate and detailed records. • Provide education and resources to patients and their families. Qualifications: • Experience in medical case management. • Strong understanding of healthcare systems. • Excellent communication, interpersonal, and organizational skills. • Ability to work independently and as part of a multidisciplinary team. • Proficiency in electronic health record (EHR) systems. • Fluency in Spanish (written and spoken) is highly preferred. Compensation: $25 - $35 yearly
