CASE ASSISTANT NEEDED
QUEENS, NEW YORK
TUESDAY - SATURDAY 9AM - 5PM
KEY ESSENTIAL FUNCTIONS:
•Provide in-home services to residents within the scope of program regulations and mission
•Carry primary responsibility for a caseload under supervision
•Provide a range of direct services, including active, hands-on skills training and supportive counseling, targeted to help residents reach wellness and community integration goals
•Provide instruction in general safety and security to all residents
•Conduct regular safety and maintenance checks of the apartments; assist in resolving facility-related problems
•Conduct apartment meetings, helping roommates live together successfully
•Ensure that residents understand the occupancy requirements and assist them in meeting them
•Complete all required documentation, such as admission package, assessments, service plans and reviews, progress notes, medication logs, walk-through and safety logs in a comprehensive and timely manner
•Conduct intake interviews
•Perform outreach and in-reach
•Assist residents in discharge planning
•Provide linkage and housing case management services
•Maintain contact with primary treatment providers or other service providers to coordinate care, attending case conferences as needed
•Involve family members in service planning, as indicated
•Escort and advocate for residents (court, school, hospital, benefits offices)
•Provide crisis management and intervention
•Attend staff meetings and trainings
•Develop, supervise and participate in resident advisory groups and program activities
•Conduct intake screenings and assessments
•Conduct aftercare for discharged residents
•Extensive travel in the community
•Provide case coverage, as needed
•Other duties as assigned
EDUCATIONAL REQUIREMENTS:
High School/GED with at least five years of experience working directly with individuals in a housing, case management or clinical program.
