Social Work Case Manager Needed
Full time-Queens 9-5 Monday-Friday

 

This position is based in a Domestic Violence Shelter. The Case Manager will be working with residents on goal definition and acquisition, housing case management, care coordination, and assistance in developing the competencies they need to live successfully in the community. The Case Manager works directly with residents to develop and implement a comprehensive support plan aimed at helping them maintain their apartment and live successfully in the community.

KEY ESSENTIAL FUNCTIONS:

• Provide case management/coordination services for a caseload of 8 families.
• Provide direct service to residents consistent with restorative services and service plan goals.
• Participate in program activities.
• Conduct group therapy with clients
• Providing documentation of services and writing progress notes.
• Conducting apartment maintenance checks.
• Complete required documentation consistent with Medicaid, OMH and company requirements and guidelines.
• Facilitate intakes and discharges.
• Develop and maintain linkages with community service providers.
• Conduct outreach; maintain contact with referral sources; maintain census.
• Work collaboratively as part of a service delivery team.

QUALIFICATIONS:

• Masters in Social Work (MSW) Degree
• A minimum of 1 year experience working with domestic violence victim is preferred.
• Excellent case work, administrative, managerial and computer skills are required.
