Social Worker-MSW- Full time position
Well staffed agency that serves the aging
Monday - Friday, 9-5 or 35 hours/week.
Manhattan Location

Tired of working long shifts with an overwhelming patient load? Come work with an organizational Case Management program where you can make a difference, working with a smaller number of patients for reasonable hours.

• Graduate of an Accredited College or University with a Master's of Social Work or equivalent Master's level degree in counseling, gerontology, psychology or related field.
• Must have experience with the geriatric population
• Good writing/record keeping skills.
• Must be computer literate, including ability to learn and use program-specific software.
• Ability to read, write, speak, and understand English.
• Ability to travel to clients' homes using mass transit, and to negotiate any steps and barriers in clients' residences.
• An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values have as they relate to serving clients and families.

• Conducting Initial Assessment and Reassessments of Home Visits
• Conducting Monitoring Phone Calls
• Providing Assistance with B/Es
• Making referrals for HDML and EISEP and HC
• Completing extensive paperwork and data entry
• Must have good computer and interpersonal skills. Also must be willing to travel within Manhattan.
