Social Worker Needed in Brooklyn!
9-5 Monday-Friday!

Responsibilities:

• Complete monthly home visits with youth, parents, and foster parents.
• Assess and provide family therapy to aid in discharge planning.
• Perform ongoing clinical assessments for children and families.
• Prepare permanency hearing reports and present in court Develop and facilitate support groups for all clients (parent, foster parents, children)
• Make recommendations for additional services when necessary.
• Attend Family Team Conference meetings, monthly case conferences, and other relevant meetings.
• Provide psycho-education to parents, foster parents, and children.
• Assist with hospitalization and discharge planning following hospitalization.
• Provide crisis intervention as needed.
• Work collaboratively as part of a team.
• Maintain progress notes and manage the case record.
• Establish intake process and ongoing monitoring of all children receiving psychotherapy.
• Involvement in existing groups.
• Group parent activities may be required.
• May be involved in agency research projects.
• May present case material at agency-wide staff meetings.

Requirements:

• MSW or MHC required. Two years' experience as a practitioner preferred.
• LMSW or LMHC required. Limited Permit accepted and LMSW/LMHC required within one year of hire. Will accept MSW/MHC contingent on taking licensing exam.
• Solid organizational and follow up skills excellent communication skills computer literacy.
• Experience with foster care or children who are at-risk highly preferred.
• Ability to manage and work with a variety of different people.
• Excellent writing and presentation skills.
• While performing the responsibilities of the job, the employee is required to frequently communicate with clients who have inquiries about services that you are providing. Must be able to exchange accurate information in these situations.
