Department: Hospice
Status: Part-time flex, 64-80 hrs per pay period
Minimum hours of position per pay period are 64 hours; Flex to 80 hours per pay period to cover planned vacations or long term absences which will be included in work schedule a minimum of two weeks in advance when possible.
Shift; M-F, days; rotating weekends, and participate in after hour call rotation
POSITION SUMMARY:Provides individualized nursing care to Hospice patients under the direction and supervision of theRegistered Nurse team.
ESSENTIAL FUNCTIONS:
Participates as a member of the Hospice Interdisciplinary Team (IDT) to assist in provision of holistic care to clients and families
Performs visits and nursing care as assigned under the supervision of Registered Nurse with
demonstrated proficiency of skills
Provides nursing care as defined in the Hospice Plan of care under the direction of Registered Nurse
Documents clinical findings and interventions provided (as detailed in the Hospice Plan of care under the direction of Registered Nurse) by the end of each scheduled shift
Demonstrates accurate and thorough documentation skills and compliance with Medicare documentation guidelines
Consults with client's nurse case manager, physician regarding clients overall needs.
Demonstrates successful management of incoming patient care related calls, assists in care
coordination with other providers as patient care needs dictate
Maintains Department productivity standards
Provides supervisory visits for Certified Nurses Aides caring for Hospice patients per Medicare guidelines
Participates in standby coverage to provide 24-hour nursing availability to Hospice clients; scheduled for standby after hour duties as required to meet patient care needs, (a minimum of once every four week nights); will work weekend shifts as necessary to meet patient care needs
Demonstrates the ability to communicate and receive constructive feedback in a positive manner
Participates in client/ caregiver education to meet Hospice patient care needs with independence
Monitor client and caregiver’s progress toward stated goals. Collaborates with Registered Nurse to adjust POC according to client response
Contributes to nursing assessment to assist Hospice team in determining level of care needed (continuous care, hospitalization, and in-patient respite)
Provide nursing visit to patient’s residence at time of client's death, determining if death has occurred using acceptable indicators such as the absence of Respirations and heartbeat as defined by the Idaho Board of Nursing; as well as provide support to family/caregivers.
Provides vacation, holiday and sick relief for nursing staff above regularly scheduled shifts as needed
Cross-trains to Home Health as required and regularly works in Home Health as department needs dictate
JOB SPECIFICATIONS:
Skills: High level of written and verbal communication skills; Ability to read, write legibly and comprehend the written and spoken word in English; Basic computer literacy and ability to effectively learn new software programs required; High level of counseling skills; Excellent relationships with clients and medical professionals; Knowledge of Hospice philosophy, Home Health care medical system, and hospital discharge planning is preferred
Education: Graduate from an approved professional school of nursing; Hospice training as provided by BGH and a professional interest in working with the terminally ill and their families
Experience: Documented evidence of a minimum two years professional experience and ability in direct patient care; Medical/Surgical experience preferred
Licenses needed: Current Licensed Practical Nurse license through the Idaho Board of Nursing; Current BLS (AHA) and valid Driver’s License required
Requirements of Employment: Personal vehicle for on-the-job transportation in all road and weather conditions, climate; Provide proof of liability insurance on personal vehicle; Must be able to successfully pass a driver’s license record background check; Employment is contingent upon successfully passing a fingerprint-based criminal history background check through the Idaho Department of Health and Welfare
