    The Licensed Practical Nurse (LPN) – Objective functions as a team member in the home health setting under the general supervision of the registered nurse. In basic patient situations the LPN is able to perform basic nursing care, record the nursing care given and report to the appropriate person changes in the condition of the patient. In basic patient situations the L.P.N. is also responsible to consult when the LPN knows or should know a delegated act may harm the patient. Additionally, the LPN may serve under direct supervision of a registered nurse, physician, podiatrist, dentist or optometrist in complex patient situations. Through direction from the Home Health Triage/Scheduling Nurse, the Registered Nurse, and/or the Nurse Manager, the LPN is assigned to make specific home visits and perform direct patient and family care in the patient’s home. S/he performs nursing interventions and personal care with patients/families with complex nursing and medical conditions in situations that require skilled technical care but do not require the direct supervision of the registered nurse. S/he works under the general direction of the registered nurse and according to a plan of care which has been created by the registered nurse. A registered nurse is available to the LPN for telephone consultation 24 hours per day, 7 days per week. The LPN knows and applies preventive nursing measures to patients with various nursing and medical conditions. The LPN demonstrates the ability to collect information, observe signs and symptoms that indicate changes in the physical/mental condition of the patient, and report appropriate information to the registered nurse. This includes reporting findings to the physician and recording new verbal orders. S/he assists patients to increased independence and self-reliance as their condition allows. The LPN is under the supervision of a Nurse Manager.
