    Bridge Community Health Clinic has an immediate opening for a full-time Behavioral Health Care Coordinator. Weekdays Only! Monday through Friday, 8AM to 5PM.

    You will be a core member of a growing integrated care team. As a behavioral health care coordinator, you will support primary care, behavioral health, and dental integration efforts through clinical and administrative duties. You will support patients and clients in meeting their care needs through direct and indirect patient/client interaction and by utilizing internal and external resources.

    These are some ways that you will make a difference:

    Provide Integrated Behavioral Health Services within an established scope of practice under direction of department leadership and assigned providers.

    Coordinate care and provide ad hoc and ongoing case management with internal and external providers and care team members for patients with complex needs.

    Conduct warm handoffs for behavioral health referrals in primary care and dental clinics.

    Connect people to healthcare and social services through referrals, warm handoffs, and follow-up.

    Coordinate incoming referral process and gather initial information to etablish care for behavioral health services in cooperation with clinic staff.

    Maintain follow-up care with patients through phone contact and office visits, while at the same time encouraging and fostering self-management.

    Provide psychoeducational and brief behavioral health interventions under appropriate supervision and direction.

    Assist with the assessment of psychosocial whole person needs and social drivers of health.

    Maintain patient files and records in accordance with regulation and policy.

    Document clinical data and patient interactions in accordance with professional standards.

    Provides education and resources for primary care and dental teams related to mental health topics.

    Advocate for patient access to benefits and services.

    Assist patients in completing paperwork and applications.

    Assist in data collection and monitoring in coordination with department leadership.

    Maintain library of community resources for patients and providers.

    Attend required meetings and participate in clinic committees and projects as requested.

    Assist the clinic with ongoing clinical projects relevant to behavioral health, integrated services, and social drivers of health.

    Answer telephone and appropriatly triage and refer callers to appropriate resource.

    Provide administrative support for integrated care and behavioral health priorities

    You will need to have:

    Bachelor’s degree in human services or related field

    Two years of experience working in a similar medical or behavioral health role

    Strong customer service skills, with problem solving experience.

    Clerical, administrative, or reception experiences.

    Having the following is a plus:

    Experience working as part of a care team.

    Experience working with an electronic health record system (EHR).

    Proficiency in Microsoft Word, Excel, Teams, and Outlook.
