    Bridge Home Health & Hospice is currently looking for an experienced, talented and dynamic Hospice Patient Care Coordinator.     The Patient Care Coordinator is responsible for providing direct assistance to the Director of Nursing/Patient Care Services for any clinically related administrative tasks. They will serve as a liaison between the clinical field staff, the Director of Nursing, and the office/admin staff. The Patient Care Coordinator will use their clinical background to make clinical decisions under the guidance of the Director of Nursing.

    Job Responsibilities

    Assist with IDT preparation and scribe during the team meeting.

    Manage the C2 request process.

    Handle death worksheets and death attestations.

    Manage the administrative duties for the Quality Assurance Process Improvement Program.

    Facilitate Field chart reviews to ensure compliance.

    Responsible for reviewing and approving the clinical supply process.

    Assist with clinical-related calls from field staff during business hours.

    Act as Administrator-On-Call one week per month.

    If necessary, provides direct patient care as defined in State’s Name Nurse Practice Act.

    Provides accurate and timely documentation consistent with the plan of care.

    Uses equipment and supplies effectively and efficiently.

    Participates in personal and professional growth and development.

    Performs other duties as assigned by the Director of Nursing.

    Required Skills

    Demonstrates good verbal and written communication, and organization skills.

    Previous healthcare administrative experience.

    Preferred Requirements

    Hospice experience

    CA state licensed LVN
