  • Responsibilities

    Job Description

    Responsible for patients referred to the Organization; identifies insurance coverage, plans and coordinates referrals for home health services. Responsible for establishing and maintaining positive relationships with customers and referral sources within an assigned territory to promote home health services.

    1. Attains a completed copy of the patient’s history and physical, visit notes, orders and any documentation pertaining to the need for home health services. 

    2. Identifies and verifies insurance coverage of home health care services. 

    3. Maintains timely response to on-line referral platforms. ( Curaspan,Enso, Allscripts etc) 

    4. Obtains and documents prior authorization for home care services from insurance providers.

    5. Assists in obtaining supplemental managed care orders under the direction of the HMO Intake Manager.

    6. Collaborates with the hospital's discharge planning personnel, utilization review department and insurance Case Managers to facilitate safe, early discharge.

    7. Consults with physicians, nurses, social workers, discharge planners and other disciplines to establish a coordinated home plan of care. Send completed patient visit notes as requested.

    8. Completes and assist referral process for Durable Medical Equipment (DME) and other related providers. 9. Interviews the patient, family, and caregiver and discusses the home situation, current needs, and any psychosocial factors that are relevant to the plan of care.

    9. Completes referral information that includes intake data, essential background information, hospital course, and the plan of care.

    10. Maintains liaison relationship with hospital and insurance personnel, providing and education on Organization services, coverage issues and related areas.

    1. Self-directed and able to work with minimal supervision. 

    2. Demonstrate excellent observation verbal and written communication skills.

    3. Exercise professional organization skills.

    4. Maintain drivers license and car insurance policies
