    Job Description

    JOB RESPONSIBILITIES

    • Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.
    • Works with physicians, nurses, social workers and discharge planners to establish a coordinated plan of care.
    • Obtains and documents prior authorization for services from insurance providers.
    • Ability to demonstrate compassion and courtesy when dealing with difficult situations.
    • Capable of working under pressure and time constraints including overtime to complete daily assessments.
    • Ability to use sound judgment and tactful approach when dealing with co-workers, staff and the public.
    • Must possess the ability to use and apply clinical knowledge to the referral process.
  • Qualifications

    Qualifications

    REQUIRED SKILLS

    • Minimum of one year of experience in intake coordinating.
    • Self-starter who enjoys working in a fast-paced environment with the ability to multi-task.
    • Excellent problem-solving, observation, written and verbal communication skills.
    • Knowledge of overall home health management including admission criteria, nursing process and insurance limitations.
    • Basic knowledge of insurances including Medicare/Medi-Cal and HMO limitations.
    • Effective interpersonal communication skills.
    • Fluent knowledge of English, both verbal and written.
