Social Work Assistant

Shift:Full time

The social work assistant will work with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided in accordance with the National Association of Social Workers (NASW) Code of Ethics along with strict adherence to government regulations. The nursing home social worker is responsible for fostering a climate, policies and routines that enable residents to maximize their individuality, independence and dignity. This climate shall provide residents with the highest practical level of physical, mental and psychosocial well-being and quality of life.

Responsibilities:

• The Social Work Assistant assists with planning and implementing a comprehensive Social Services program which provides for counseling and other support services for customers and legal representatives in the Center.
• Identifies medical-related social needs of customers, provides appropriate services to meet the individual, as well as collective needs of customers, and maintains records relating to the customers’ social work needs and care.
• Consults with the Director of Social Services and other department heads regarding interdisciplinary issues, as well as maintenance of appropriate records.
• Works directly with customers and legal representatives experiencing personal and environmental difficulties or concerns related to the customer’s physical or emotional condition.

Requirements/Qualifications:

• Bachelor’s in Social Work is preferred. A Bachelor’s Degree in human services may also be considered.
• Previous experience in the field required. Previous long term care experience strongly preferred.
• The Social Work Assistant needs to possess any Certifications/Licensures required by State regulations.
