    GENERAL DESCRIPTION

    To provide episodic care to individuals served living in the ACCS  residences and community.  The scope of the position is to provide assistance to person’s served with daily living task under the supervision of a registered nurse.

    ESSENTIAL JOB FUNCTIONS

    • Develop and assist with a person center planning around basic care services for hygiene that will include bathing, dressing, oral care, shaving, hair and nail care.
    • Facilitate open communication with in the teams concerning persons served.
    • Assist/administer person served in taking medication according to MAP guidelines
    • Develop and support in a person centered manner transportation options and abilities, which may include providing transportation.
    • Support person served needs for community integration and self directive skills.
    • Update all person served records as required and review and document person served progress towards goals listed in the Treatment Plan daily
    • Assist/administer person served in taking and applying topical medications according to MAP guidelines.
    • To assist with person centered assessments and care planning
    • Check and record all vital signs, IE height, weight, Blood Sugar levels, Blood Pressure and Pulse.
    • Continually engage and instill a sense of hope with person served
    • Facilitate personal wellness, including exercise, smoking cessation, nutrition and spirituality.
    • Participate in all medical team meetings including weekly supervision, keeping supervisor well informed and update collateral services when needed.
    • Participate in all relevant staff meetings, recovery team meetings, as needed.
    • Work in collaboration with the RN and the psychiatric team.
    • Communicating effectively in writing and orally to the ACCS  team.
    • Assist person served with medical and psychiatric appointments.
    • Serve on interagency committees
    • Keep supervisor well informed and update collateral services when needed
    • Report any incidents, accidents, injuries or medication errors to your supervisor according to agency policy
    • Other duties as assigned by supervisors

    OTHER RESPONSIBILITIES:

    • Flexibly respond to program needs throughout Berkshire County
    • Represent the agency in a professional manner in all community contacts
    • Encourage independence and model enthusiasm, positive community interaction and spirit of recovery
    • Stay awake and alert during all shift hours
    • Possible weekend and holiday coverage

    QUALIFICATIONS:

    • High School Diploma or GED required
    • Current Certified Nursing Certificate in the Commonwealth of Massachusetts.
    • Ability to write clear concise reports incorporating observations of persons served behavior
    • Ability to make some independent decisions based on defined recovery goals
    • Experience in one or more of the following areas: wellness, person centered planning, employment, education, human rights, housing, benefits and entitlement programs preferred
    • Knowledge of licensing requirements and the ability to maintain those standards
    • Relevant experience preferred, will train
    • Proficient computer skills in using web based e-mail and ability to data enter information into spreadsheets and word processing documents
    • MAP and CPR  and First Aid Certified, or obtain MAP and CPR and First Aid certification within first 3 months of employment
    • Able to pass a Criminal Offender Record Information Check and RMV check
    • Drivers License and ability to use a personally insured vehicle for agency business

    SKILLS:

    • Ability to write clear concise reports incorporating observations of persons served behavior
    • Ability to make some independent decisions based on defined recovery goals
    • Experience in one or more of the following areas: wellness, person centered planning, employment, education, human rights, housing, benefits and entitlement programs preferred
    • Knowledge of licensing requirements and the ability to maintain those standards

    WORKING CONDITIONS:

    • Work at various residential sites and person’s severed community living.
    • Able to work or travel inside and outside as needed in varying weather conditions in person served community living environments with varied conditions

    PHYSICAL REQUIREMENTS AND CONDITIONS

    • Manual & visual dexterity; correctable
    • Lifting Maximums: weight of grocery bags and/or laundry baskets
    • Push, pull, stand, reach overhead and twist
    • Able to bend at the knees, waist and lower self to the ground or floor without assistance
    • Able to physically climb and descend stairs without assistance
