Clinical Care Coordinator Responsibilities
Provides care as defined within the scope of practice
Makes the initial evaluation visit and regularly reevaluates the home care client’s nursing needs
Prepares the clients initial assessments and plan of care
Completes regular supervisory visits and reassessments on clients and care providers
Assists with in-service and training programs
Performs skilled services such as injections, wound care, medication set up, cath care, etc.
Clinical Care Coordinator Requirements
Current unrestricted RN license
Reliable car/full coverage auto insurance
Clean background and criminal record
Understands regulatory compliance with state and federal regulations
Adhere to HIPAA and maintain client confidentiality
Demonstrated ability to supervise and direct staff.
Possess exceptional comprehensive nursing assessment skills
Comfortable with various computer systems and electronic medical records, data entry, email
Comply with accepted professional standards of practice.
Treat clients, staff and the public with courtesy, respect and presents a positive public image.
