Direct Care Residential Mentor Job Description: 21 yrs of age minimum requirement

The primary responsibility is to provide support and care for the children living in the home and to

maintain a safe environment. Mentors assist the children by directing interactions, including but not

limited to behavioral management, and by facilitating therapeutic recreational and social life skills.

Additional Responsibilities include but are not limited to:

Completing ongoing training, including CPR, First Aid, and CPI

Participating in monthly team meetings and agency-wide training

Assisting with client admission and discharge

Following an assigned schedule for daily activities

Overseeing clients’ completion of daily living tasks

Follow house rules and schedules to provide consistency

Dispense medications

Complete daily progress notes

Safely and appropriately restrain clients using CPI standards if necessary

Maintain all DCS licensing standards required by Article 74.

Qualifications: To qualify for a Residential Youth Care Mentor you must possess the following qualifications and documents (We can help direct you to get the qualifications you need).

FINGERPRINT CLEARANCE CARD (CODE: A.R.S 46-141 LEVEL 1)

CPR & FIRST AID (IN-PERSON CLASSES ONLY)

HIGH SCHOOL DIPLOMA/G.E.D. OR HIGHER

CPI CERTIFIED (CRISIS PREVENTION INTERVENTION): (WE PROVIDE TRAINING)

INITIAL HEALTH SCREENING TRAINING (WE PROVIDE TRAINING)

VALID ARIZONA DRIVER'S LICENSE (NO RESTRICTIONS)

PERSONAL VEHICLE WITH VALID AUTO INSURANCE & REGISTRATION

PHYSICAL EXAMINATION (COMPLETED WITHIN 6 MONTHS)

3 NON-RELATIVE REFERENCE LETTERS

2 YEARS PROFESSIONAL REFERENCES

ADMINISTRATION WRITING SKILLS
