    Please note- This is a NIGHT SHIFT position with a Hospice Organization.

    What you will be doing:

    • Perform comprehensive assessment of patient and address any immediate needs.
    • Revise and document patient’s Plan of Care (POC) as appropriate.
    • Accurately and fully document in the patient’s electronic medical record (EMR) as required.
    • Prepare, administer, and document medications and treatments as prescribed. Educate patient and family self-care techniques.
    • Identify patient/family health and social issues and potential problem areas.
    • Implement measures to address identified patient/family needs.
    • Collaborates with primary care physician, specialist, hospitalist and medical directors.
    • Communicate pertinent clinical findings to physician, supervisor, and staff as appropriate.
    • Perform the admission process as required including: 1.) Explains components of client’s program to patient/family and primary caregivers and obtain consent for care. 2.) Discuss with patient/family their needs and expectations and our client’s capabilities and limitations in meeting them. 3.) Completes all admission forms including required signatures.
    • Comply with all regulatory compliance requirements and deadlines.

    A successful candidate will possess:

    • Graduate of an accredited nursing school; Bachelor’s degree preferred
    • Registered Nurse with current Ohio license
    • Two year’s clinical experience, preferably in hospice, homecare, oncology, gerontology, or surgical nursing
    • CPR Certified
    • Valid Driver’s License
    • State Minimum Auto Coverage
    • Competency in information technology: Experience with an Electronic Medical Record (EMR), Navigating Windows, Word, E-mail, and other programs which may assist with the operation and communication of the organization.

    Expected Hours of Work:

    This position operates in a healthcare environment open 24 hours a day, 365 days a year. Generally scheduled Monday through Friday. Required to fulfill shifts for after-hours scheduling (evenings), weekends and holidays.

    Work Environment:

    This position operates in healthcare settings found in personal homes, nursing facilities and hospitals. May encounter exposure to blood/body fluids.

    Travel:

    Daily travel is required for this position. Travel may be during daytime, nighttime or inclement weather.

     

    Physical Demands:

    The physical demands described here are representative of those that might be met by an employee to successfully perform the essential functions of this position. While performing the duties of this position the employee is regularly required to talk and hear. This position is very active and requires standing, walking, climbing, bending, kneeling, twisting, and stooping all day. The employee must frequently lift or move items weighing over 50 pounds.
