Caregiver Job Description (Home Health & Hospice)
Position Summary
The Caregiver provides essential non-medical and personal care services to patients in their homes or hospice settings under the supervision of a Registered Nurse (RN) or clinical care team. The caregiver supports patients with activities of daily living (ADLs), promotes comfort, safety, dignity, and emotional well-being, and ensures quality care in accordance with the patient care plan.
Key Responsibilities
Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility
Provide personal care and hygiene support in a safe and respectful manner
Assist with feeding and meal preparation as needed
Monitor and report changes in patient condition to the supervising nurse
Provide companionship and emotional support to patients and families
Assist with light housekeeping tasks (laundry, cleaning patient area, changing bed linens)
Support patients with safe transfers and mobility assistance (walker, wheelchair, transfer belt)
Follow care plan instructions developed by the RN or care team
Maintain patient confidentiality (HIPAA compliance) at all times
Document care activities as required by agency policy
Follow infection control and safety procedures (OSHA standards)
Communicate effectively with RN, case manager, and interdisciplinary team
Required Qualifications / Skills
Compassionate, patient, and caring attitude toward elderly and disabled patients
Ability to follow written and verbal instructions
Good communication and interpersonal skills
Physical ability to lift, move, or assist patients as required
Reliable transportation (if required for home visits)
Ability to work independently in patient homes
Understanding of basic infection control and safety practices

Minimum Requirements
High School Diploma or GED (required)
