    Job Description

    • Assess housing needs and develop objectives and recommendations for ACT treatment services to support consumers to secure housing. 
    • Coordinate with ACT Team Intake Specialist to identify housing risk and needs of incoming participants prior to their release from jail.
    • During the assessment obtains information: a) to establish the participant’s housing needs and preferences; b) to prepare consumer to access appropriate and available supportive housing
    • Complete ACT comprehensive assessment and six-month updates
    • Act as advocate and liaison for participant in securing supportive housing 
    • Complete housing applications (2010e) and establish tracking mechanisms to monitor consumer access to housing and ensure at least 10-15 consumers admitted to the program each year achieve supportive housing within 3-4 months of release from jail
    • Identify the treatment services consumers should receive in the program  based on problems, strengths and preferences and immediate needs
    • Assist participant in the development of housing goals and modify the goals in the light of feedback from recipient, family members and the team
    • Develop comprehensive service plans and 6-month service plan reviews in collaboration with the consumer and the consumer’s support system
    • Build in review and evaluation points, and modifies the service plan in the light of feedback from recipients and relevant others
    • Provide individual supportive counseling and problem-solving as needed to consumers and supportive housing providers to ensure consumers are retained in housing.
    • Involve family and significant others in the consumer’s treatment with the ACT team
    • Complete 6-month comprehensive assessment and crisis intervention and relapse prevention plans, progress notes and significant data
    • Work effectively with community housing providers, education and vocational training providers, and self-help groups providing services to ACT consumers.
    • Update and track data for participants receiving housing related services
    • Complete monthly and annual PATH reports
    • Provide 24-hour crisis intervention on-call services on rotating basis
    • Provide 80 percent of treatment contacts in the community
    • Provide culturally competent services in accordance with CASES policies
  • Qualifications

    Qualifications

    • At least three years of experience working in human services with homeless adults with serious mental illness and those with serious mental illness and co-occurring substance use disorders and involvement in the criminal justice system.  
    • Experience understanding various evidence-based models and theories pertaining to treatment of substance abuse, mental illness, and risk, need, responsivity to criminal behavior
    • Extensive knowledge of Assertive Community Treatment, community treatment, support services, and resources 
    • Highly organized and self-motivated
    • Proficient in computers 
    • Fluency in Spanish preferred
