About the Role:
Aura One Hospice is looking for a compassionate and dedicated Certified Home Health Aide (CHHA) to join our team in Palm Desert, CA. Responsibilities:
Provide personal care services to hospice patients in the home setting, including bathing, grooming, and hygiene assistance
Assist patients with mobility, transfers, and ambulation as directed by the care plan
Monitor and document patient condition changes and report observations to the supervising nurse
Support patients with light housekeeping, meal preparation, and activities of daily living (ADLs)
Deliver compassionate, patient-centered care in alignment with hospice philosophy and end-of-life care standards
Communicate effectively with interdisciplinary hospice team members, patients, and families
Maintain accurate documentation and adhere to care plan guidelines
Requirements:
Valid California Certified Home Health Aide (CHHA) certification required
Prior experience in hospice, home health, or palliative care preferred
Current CPR/First Aid certification
Reliable transportation and valid driver's license
Strong interpersonal skills with a compassionate, patient-focused demeanor
Ability to work independently and as part of a collaborative care team
Comfortable working in home environments with patients facing end-of-life care needs
