    Job Summary

    The primary responsibility of this professional is to provide targeted (intensive) case management services to the family of the identified client. The following list identifies skills necessary for effective Wraparound Care Coordination. This list of skills should be conceptualized as a tool kit for Care Coordinators. The list is divided into the four phases that typically occur in Wraparound, including Engagement, Planning, Implementation and Transition. This professional is also to be trained in family oriented, individual and/or group rehabilitative mental health services, skills training and development, medication training, parenting skills training, and case management services to clients of the agency per state specified standards and requirements. It is the responsibility of the professional to provide targeted (intensive) case management services a minimum of fifteen (15) hours per week to a maximum of ten (10) families.Must possess a minimum of a bachelor’s degree in rehab counseling, psychology, social work, or related field as specified in the Texas Administrative Code for QMHP qualifications. General understanding of adult and child mental health disorders, treatment of disorders, and documentation of treatment provided. Knowledge of confidentiality and ethical guidelines for practice. Should possess basic therapeutic skills, verbal and written communication skills, and organizational skills. Trained in working with individuals with Severe and Persistent Mental Illness, Children with Severe Emotional Disturbances, and delivery of services based on curriculum.

    Responsibilities

    Provide targeted (intensive) Case Management services to families

    Provide and exhibit Engagement, Planning, Implementation and Transition skills as identified by the Wraparound Care Coordinator Tool Kit

    Reports suspected child abuse or neglect immediately to the Abuse and Neglect Hotline and Director

    Ensure compliance with all rules and regulations of DSHS including licensure rules and regulations and suspected fraudulent practices

    Responds to on-call cell phone immediately

    Attend all Team, Individual, and Training Meetings as required and scheduled

    Document services provided with the understanding that all clinical notations are considered legal documents and can be used in a court of law. All documentation should be true and accurate and under no circumstance falsified in any manner

    Maintain client confidentiality at all times and be familiar with all HIPPA laws and regulations

    Prepare and present all assigned cases for review by a supervisor, as required on a prescribed basis

    Opens and processes mail / e-mail daily in a timely manner

    Participate in Child and Family Team meetings as appropriate

    Provide transportation as required

    Qualifications

    Must possess a minimum of a bachelor’s degree in rehab counseling, psychology, social work, or related field as specified in the Texas Administrative Code for QMHP qualification and have at least one year of related experience.

    Requires flexibility in scheduling with evening and weekend hours

    Able to concentrate on fine detail with some interruption and to focus attention on tasks for 45-60 minutes at a time on a continuous basis

    Professional demeanor and appearance required; willing and able to comply with Covenant Kids Professional Appearance in the Workplace policy

    Able to communicate effectively with people and to assess family strengths and deficits

    Able to handle multiple tasks; a self-starter who is energetic and detail-oriented with excellent organizational skills

    Able to communicate effectively both verbally and in writing

    Able to deal with a variety of adults and children with a range of personalities and problems

    Able to maintain a high level of confidentiality
