    Position Summary

    The Certified Nursing Assistant (CNA) provides hands-on personal care and support to hospice patients in their place of residence. This role focuses on maintaining patient comfort, dignity, and quality of life while working closely with the interdisciplinary team.

    The ideal candidate is compassionate, reliable, and attentive to patient needs, with a strong commitment to providing comfort-focused care.

    Key Responsibilities

    Patient Care & Support

    Provide direct personal care to patients, including:

    Bathing, grooming, and hygiene care

    Dressing and toileting assistance

    Mobility support, transfers, and repositioning

    Assist patients with activities of daily living (ADLs)

    Maintain a clean, safe, and comfortable patient environment

    Observe and report changes in patient condition to the RN Case Manager

    Care Coordination

    Follow the established Plan of Care under the supervision of the RN

    Communicate effectively with the interdisciplinary team (IDT), including:

    RN Case Managers

    Social Workers

    Chaplain/Spiritual Care

    Support patients and families with compassionate presence and care

    Documentation & Compliance

    Complete CNA visit notes accurately and within required timeframes (24–48 hours)

    Maintain proper documentation in the EMR system

    Follow hospice policies, procedures, and infection control standards

    Support for Patients and Families

    Help families understand basic care needs and comfort measures

    Qualifications Required

    Active Certified Nursing Assistant (CNA) certification in Texas

    Minimum 3-5 years of CNA experience (hospice or home health preferred)

    Valid driver’s license and reliable transportation

    Ability to work independently and as part of a team

    Strong communication and time-management skills

    Preferred

    Hospice or home health experience

    Experience with EMR systems (e.g., Kinnser/WellSky)

    Understanding of hospice philosophy and comfort care
