Job Summary
Provides personal care services under the direction of the Registered Nurse . The Aide is assigned to specific clients by the Registered Nurse or other appropriate professional who performs services for clients as necessary to maintain their personal comfort and well-being.
Reports To: Clinical Supervisor Provides Supervision To: NA – No supervision responsibilities
Responsibilities
Provide companionship and, if authorized, transportation to clients according to the care plan.
Assist client with general Activities of Daily Living (ADLs) such as bathing, meals, and safe ambulation/transfers.
Report any changes in the client’s condition to the supervisor as outlined in the care plan.
Document care provided and maintain accurate records for the Agency.
Work collaboratively with the client’s care team and ensure effective communication.
Promote client safety by following infection control practices and reporting unsafe conditions.
Attend in-service programs to meet compliance and notify supervisors of any educational needs.
Qualifications – Certified Nursing Assistant (CNA) • Must meet one of the following: • Possess a current and active CNA certification issued by the state of Florida. • Provide proof of successful completion of a state-approved CNA training program. • Submit proof of graduation from an accredited school of nursing (Florida or other state). • Must be at least eighteen (18) years of age. • Ability to read, write, and follow a written care plan effectively. • Demonstrates strong verbal, written, and interpersonal communication skills. • Must be a licensed driver with a personal automobile that is: • Properly insured in accordance with state and/or Agency requirements. • In good working condition. • Hold a current CPR certification from an accredited provider.
