    Job Responsibilities

    • Coordinate and support CNA services for home health and hospice patients.
    • Assist with CNA scheduling, coverage, and communication between staff and clinical leadership.
    • Serve as a liaison between CNAs, RN Case Managers, and office staff.
    • Provide guidance, support, and ongoing communication to CNA team members.
    • Assist with onboarding, training, and competency support for CNAs.
    • Perform occasional home visits to assist patients with activities of daily living as needed.
    • Monitor documentation to ensure visits and services are accurately charted and completed timely.
    • Recognize patient needs, emergencies, and changes in condition, and report appropriately to RN Case Managers.
    • Ensure compliance with organizational policies, documentation standards, and professional conduct expectations.
    • Promote a safe, healthy, and professional environment for patients and staff.
    • Assist with additional office and administrative tasks as needed to support daily operations.

    Job Requirements

    • Current Utah Certified Nursing Assistant (CNA) certificate.
    • Minimum 2 years of experience as a CNA preferred. Active patient contact within the past three years preferred.
    • Current driver’s license, auto insurance, and reliable transportation.
    • Leadership or coordination experience is preferred.
    • CPR Certification.
    • Strong organizational, communication, and leadership skills.
    • Ability to problem-solve, multitask, and work independently.
    • Ability to transfer and position patients safely when providing direct care.
    • Must be at least 18 years old.
