Client Care Plan / Caregiver Assignment
Female Client
Age: 74
Primary Condition: Bed-bound due to spinal bleeding ulcer resulting in rheumatoid arthritis affecting the lower back and left knee.
Mobility Status: Non-ambulatory (unable to stand or walk). Requires full assistance with transfers and repositioning.
Personal Care Assistance
Caregiver will assist the client with the following daily care needs:
Assist with repositioning from side to side in bed to prevent pressure sores.
Provide daily bed bath.
Assist with personal hygiene and grooming as needed.
Monitor skin condition and assist with pressure sore prevention.
Transfer & Mobility Assistance
Client requires use of an Electric Hoyer Lift for transfers.
Caregiver must be comfortable assisting with transfers using lift equipment.
Client weighs approximately 260 lbs, therefore caregiver must be able to safely assist with lifting and repositioning.
Client will assist when possible during care.
Scheduled Out-of-Bed Routine
Client gets out of bed three times per week:
Days: Monday / Wednesday / Friday
Duration: Approximately 2 hours
During this time caregivers will:
Transfer client using Hoyer Lift.
Change draw sheets and bed linens.
Assist with laundry (wash and dry).
Housekeeping Responsibilities
Light housekeeping limited to the client’s bedroom:
Sweep floor daily
Mop client’s room once per week
Maintain a clean and safe environment
Medical Equipment
Client sleeps on a therapeutic pressure-relief mattress.
Mattress must be rotated/adjusted every other day to prevent bed sores.
Medication Assistance
Caregiver may be asked to pick up medications from Kroger Pharmacy, located near the client’s home.
Meal Assistance
Meals are prepared by the client’s sister and stored in the refrigerator.
Caregiver will warm meals in the microwave when needed.
Household Support
Client’s nephew handles grocery shopping and supply pickup.
Gloves are available in the home, but caregivers may bring their own if preferred.
Care Orientation
The client will provide instruction on her care routine upon caregiver arrival until the caregiver becomes familiar with the process.
Additional care instructions and expectations will be discussed during the first visit.
