  • Responsibilities

    Reporting to the Administrator/Vice-President of Skilled Nursing Operations, the Assistant Director of Health Information is responsible for the overall operation of the Skilled Nursing and Rehabilitation Center Health Information Department.

     

    EDUCATION:

     

    Certification as Registered Health Information Technician (RHIT) or Bachelor's degree as Registered Health Information Administrator (RHIA) required.

     

    WORK EXPERIENCE:

     

    Minimum of three (3) year experience in a medical records setting required.

    Successful completion of the registration examination conducted by the American Health Information Management Association required.  RHIA or RHIT with (3) years experience.

    Leadership ability to recognize problems, make decisions, and use judgement in applying standards, procedures, and policies required.

    Familiarity with DOH regulations related to medical records, health Information regulations and legal aspects of Health Information required.

    Supervises Health Information staff in their day-to-day work activities consisting of; coding, transcription, release of information, and technicians/clerks.  Consistently guides, directs and counsels staff members regarding work performance, problem solving and decision making ability. 

    Assists in development of department goals and objectives. Establishes and maintains the department in an innovative, efficient, and cost-effective manner. Formulates  policies and procedures of the department and ensures proper implementation with Administrator approval.  Effectively communicates changes in policy, procedure and other matters to staff and provides feedback from staff to Administrator. 

    Responsibilities include but are not limited to medical staff credentialing, auditing physician visit compliance, compliance of physician orders, and MDS Coding.(

    Conducts performance/process improvement studies along with productivity monitoring for the HIM staff.  Consistently works with staff regarding cross-training and achieving great efficiency within the HIM job duties.  Collects and compiles the data submitted by the HIM staff. 

    Plays a key role in implementing medical record applications/software.  Achieves a thorough knowledge of the medical record applications/software directly involved with HIM workflow.  Assists in educating and training the HIM staff on all HIM applications/software along with being that go to vital resource for the HIM staff.  

    Develops and maintains a coding audit/compliance process in conjunction with a physician query process.  Compiles  and reports the coding audit data quarterly to the HIM Director for the Corporate Compliance Committee.  Provides feedback monthly to the Coders regarding their coding audit(s) and identifies  the need for further Coder education and will determine areas requiring focus/improvement.   Compiles and reports the physician query data monthly to the HIM Director for the UR/PPS Committee. 

    Develops and maintains a document capture quality audit/compliance process.  The document capture consists of scanning, quality control and validation. Compiles and reports the document capture quality audit data quarterly to the Administrator  for the Corporate Compliance Committee.    Provides feedback monthly to the Technicians regarding their document capture audit(s) and identifies  the need for further education and will determine areas requiring focus/improvement.   

    Plays a key role in focused medical record documentation reviews in order

    to maintain survey readiness for  SNF  surveys.

    Develops and maintains telecommuting programs.  The programs consist of coding, medical transcription and chart analyst.  

    Answers request for information according to the release of information policies and procedures.  Directly involved with training of HIM personnel utilizing our release of information application Medical Records Online

    Completes MA-51 Medical Evaluation Application in absence of coding staff.

    Ensures employees are scheduled and paid within the Kronos timekeeping system.  Reviews and evaluates all requests for time off.  Accurately completes and approves timecards.  Monitors and evaluates department objectives to ensure that time off is used effectively in order to manage departmental work flow. 

    Assists in evaluation of personnel in performance appraisals.  Makes effective recommendations in matters of hiring, promotion, discipline and termination. 

    Performs ICD -9-CM / ICD-10-CM coding of inpatient and outpatient medical records in the absence of coding staff. 

    Performs qualitative and quantitative analysis of inpatient medical records, in the absence of the analysis staff

    Participates in in-services and continuing education lectures and seminars.

    Screens applications, interviews, hires and orients all personnel

    At the discretion of the Administrator, attends department-head and divisional meetings and takes minutes. 

    Manages and maintains the destruction schedule of scanned paper records on a monthly basis.  Utilizes the discharge list from three months prior to reconcile inpatient rehab and transitional rehab discharges and outpatient accounts.  Compiles and reports the number of records destroyed for inpatient rehab, transitional and outpatient records quarterly to the Administrator  for the Corporate Compliance Committee.
