    JOB SUMMARY

    The Health Coach is responsible for assessing and developing self-management plans for CalOptima members benefiting from chronic condition management, nutrition management, and/or psychosocial support aligned with the member’s individual care plan. The incumbent is responsible for sharing the member’s specific self-management goals, progress, and other pertinent information with their health care team to ensure consistency of member goals. Additionally, the incumbent will facilitate the tracking of member progress and promote quality outcomes. Delivers interventions through various methods and techniques that are effective to CalOptima members. Collaborates with CalOptima providers, health networks, and community partners.

     

    POSITION RESPONSIBILITIES:

    • Develops content in simplified concepts and terminologies related to all CalOptima Population Health Management programs; utilizing clinical practice guidelines and assisting the team in the review and selection of chronic illness program resources for members.
    • Conducts assessments to evaluate member health needs. Uses standardized instruments and inputs into specified electronic data platforms, assesses results to develop self-management plan and informs the intensity of coaching support to be offered. Assessments to include:
      • Member’s educational and self-management needs aligned to the Primary Care Physician (PCP) and Individualized Care Plan (ICP), medication adherence, activation level, and a readiness to change screening.
      • Member’s health behaviors, health condition monitoring, and adherence to treatment plans.
      • The identification of the member’s strengths and support system for incorporation into the self- management plan.
    • Identifies additional external resources for members and facilitates the administration of them.
    • Monitors member’s health condition and self-management goal outcomes. Shares self- management plan with the member’s health care team as appropriate to ensure coordination of services.
    • Anticipates member needs and proactively support reducing risk for medical and behavioral health treatment at a higher level; directs members with acute clinical problems or behavioral health issues to a Case Manager from the medical group, CalOptima, and/or the appropriate Behavioral Health provider.
    • Acts as:
      • An integral part of the team providing verbal and written support interdepartmentally as well as providing support to community agencies, health networks, and social services.
      • A resource for CalOptima providers, health plans, and the community.
    • Coordinates and identifies member psychosocial needs, utilizing community resources and support when appropriate. Follows CalOptima’s protocol for documenting all interventions and completes productivity reports as required.
    • Assesses member and community needs and determines priorities for chronic illness interventions.
    • Other projects and duties as assigned.

     

    POSSESSES THE ABILITY TO:

    • Develop creative, innovative approaches to challenges and opportunities.
    • Engage and participate as a care team member working collectively toward a common set of defined goals.
    • Support behavior change, adult learning, and application.
    • Engage in motivational interviewing and client-centered counseling.
    • Establish and maintain effective working relationships with all levels of staff, other programs, agencies, contracted providers, and the general public.
    • Work within the scope of the disease management program.
    • Communicate clearly and concisely, both verbally and in writing, with individuals from diverse backgrounds.
    • Utilize computer and appropriate software (e.g., Microsoft Office: Word, Outlook, Excel, PowerPoint) and job specific applications/systems to produce correspondence, charts, spreadsheets, and/or other information applicable to the position assignment.

     

    EXPERIENCE & EDUCATION:

    • Bachelor’s degree in Health Science, Nursing, Nutrition, Public Health, Social Work, or another related field required.
    • Individuals with a clinical background, a current unrestricted Registered Nurse (RN) license, Certified Physician’s Assistant (PA-C), or Registered Dietitian license to practice in the State of California is required.
    • 2 years of experience in health education or health coaching, preferably in a managed care environment, required.
    • Bilingual in English and in one of CalOptima's defined threshold languages (Spanish, Vietnamese, Arabic, Farsi, Korean, Chinese) is required.

     

    PREFERRED QUALIFICATIONS:

    • Master’s degree in Health Science, Nursing, Nutrition, Public Health, Social Work, or another related field preferred.
    • Preferred active certifications include Certified Case Manager (CCM), Chronic Care Professional (CCP), Certified Diabetes Care and Education Specialist (CDCES), Certified Health Education Specialist (CHES), Asthma Educator-Certified (AE-C), and/or Master Certified Health Education Specialist (MCHES).

     

    KNOWLEDGE OF:

    • Health literacy and plain language communication.
    • Appropriate techniques, practices, and cultural sensitivities to serve diverse groups.
    • Principles and practices of population health management.
    • Principles of evidence-based medicine and measurement against clinical practice guidelines.
    • American Disability Act (ADA) requirements and Community Living Assistance Services and Support (CLASS) standards.
    • Principles of managed health care service delivery.
