Remote LPN – Patient Outreach Coordinator (Contract)

We are seeking a licensed LPN for a remote patient outreach role supporting home and telehealth visits.

Details:

• Schedule: Full-time, 40 hrs/week (9am–6pm EST)

• Location: Remote (U.S. only)

• Contract position

• Interview: Video via Microsoft Teams

Responsibilities:

• Outbound calls to scheduled patients

• Phone-based patient assessments

• Chart preparation and EMR documentation

Requirements:

• Active LPN license

• EMR experience (eCW preferred)

• Strong communication skills
