    JOB SUMMARY

    The Advance Practice Registered Nurse (APRN) will oversee admissions and discharges, develop the clinical care plan, and supervise the clinical care coordination for residents. Provide direct support, conduct various assessments, address illnesses/injuries and ensure proper follow-up, make referrals to other medical professionals/facilities as appropriate, serve as a member of the Interdisciplinary Team and participate in the ISP planning and implementation processes as required, ensure adequate nursing personnel is scheduled. Provide supervisory oversight to LPNs, complete nursing personnel timekeeping/related functions are completed in a timely fashion; and, provide program leadership and support.

     

    REQUIREMENTS

    • Masters or Bachelors in nursing with license as an Advance Nurse Practitioner (Family or Addiction Medicine) and licensed to practice nursing in the District of Columbia. Board Certified in Addiction Medicine or on track or desire to be board certified, preferred.

    • (2) Two years nursing experience working in the SUD field with experience in a residential setting and working with people who may have experienced chronic homelessness, co-occurring disorders, and substance use disorders.

    • Ability to document all healthcare and medical needs as required by all licensing and monitoring entities and work collaboratively on a clinical treatment team.

    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.

    • Ability to perform basic computer and office equipment skills, write routine reports, correspondence, effectively present information in one-to-one and small group situations to people supported, families, guardians, and other medical professionals.

    • Proven ability to for conflict resolutions

    • Bilingual in any language a plus.

     

    RESPONSIBILITIES

     

    • Lead and supervise clinical care coordination and LPN’s.

    • Be available, on-call and as scheduled, for nursing consultation and/or treatment of illnesses or injuries and render nursing assistance in medical emergencies.

    • Develop rapport with each person receiving nursing care. Demonstrate respectful interactions at all times.

    • Participate in the initial admission, evaluation and treatment plan designed for each person admitted to this program.

    • Provide appropriate in-service training to staff in basic health care and hygiene skills.

    • Supervise and support staff in the provision of medical support for the person.

    • Participate as a member of the Interdisciplinary Team providing general health counseling, review of medical procedures, medication and medical options with the person, their family, guardian and staff.

    • Help person and family coordinate all medical appointments.

    • Complete the following specific on-going tasks.

    • Make appropriate entries in nursing notes related to the care of the people supported.

    • Maintain all appropriate medical documentation such as medical consultations, medical history, medication errors, etc.

    • Help coordinate all hospital discharge instructions.

    • Prepare medical summaries and attend ISP meetings as scheduled.

    • Complete identified assessments as needed at least annually for each person supported.

    • Communicate with the Sr. Program Director regarding each person’s medical needs including routine, and well as, emergency treatments.

    •  

    • The APRN should have working knowledge of community resources and a plan for accessing those resources.

    • Help coordinate all nursing-related information during program discharge or transfers.

    • Maintain up-to-date knowledge of Volunteers of America Chesapeake’s policies and procedures.

    • PERFORM OTHER DUTIES AS ASSIGNED.

     

     

    Required Skills Required Experience

  • Qualifications

     

    SKILLS:

     

    • Experience in establishing programs such as work order tracking, inventory accountability, job assignment and tracking, and process controls.
    • Electrical, PLC and vertical form and fill equipment knowledge.
    • Ammonia refrigeration knowledge desirable. 
    • Computer skills using a variety of software programs.
    • Excellent interpersonal skills
    • Strong verbal and written communication skills.
