    JOB DESCRIPTION

    MDS COORDINATOR

    JOB SUMMARY:

    The overall purpose of the MDS COORDINATOR is to assure that all state and federal MDS assessments are completed and electronically submitted within the guidelines outlined by the governing agencies.

    QUALIFICATIONS:

    ◆   A current valid license to practice nursing in the state of Missouri. RN required.

    ◆   1-year long term care experience with knowledge of Medicare documentation requirements desired.

    ◆   Highly developed communication and interpersonal skills are needed.

    ◆   Ability to read, write, and communicate in the English language.

    ◆   Ability to effectively present information and respond to questions from department heads, customers, ( patients, families, physicians, etc.) and the general public.

    ◆   Computer skills. Prefer knowledge of MDS software (PCC, Ability) and electronic transmissions.

    ESSENTIAL FUNCTIONS:

    ◆   Obtains Medicare qualifying diagnosis for residents obtaining Medicare A services and updates diagnosis for each change in diagnosis.

    ◆ Prepares and/or coordinates the preparation of MDS, and Care Plan on each resident.

    ◆   Maintains schedule of MDS and care plans for timely completion.

    ◆  Coordinates weekly Medicare meetings, and family care plan conferences.

    ◆   Assures preparation of Medicare denial letters for each resident discharged from Medicare coverage within time limits imposed by Medicare guidelines.

    ◆   Assures authorizations for Managed Care are received and updates are provided in timely manner.

    ◆   Coordinates services with interdisciplinary team.

    ◆   Ability to function as a Team Leader/Role Model.

    ◆   Has reviewed Camelot’s Policies and Procedures for Abuse Prevention and knows the employee's responsibility to enforce it.

    ◆   Assist in on-call scheduling rotation.

    ◆   Assist in making transportation arrangements for residents’ appointments.

    ◆   Other duties as assigned by Administrator or DON.

    ◆   Professional contacts will include physicians, facility staff, residents, families, hospitals, and other resources, fiscal intermediaries and others.

    CONTINUING EDUCATION:

    ◆  Attends in-service and educational programs.

    ◆   Attends continuing education required for maintenance of professional certification or licensure.

    PHYSICAL DEMANDS:

    ◆   Ability to communicate in English, via phone, writing, and verbally in conversation with different levels of staff, patient families, and any outside customers.

    ◆  While performing the duties of this job, the employee is frequently required to stand and walk. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision distance vision, peripheral vision, depth perception, and ability to adjust focus. Ability to utilize a computer, phone, etc. for extended periods of time.

    ◆   While performing the duties of this job, the employee is occasionally exposed to blood or other body fluids, fumes or airborne particles and toxic or caustic chemicals. The noise level in the work environment is usually moderate.
