NOW OFFERING A NEW AFTER-HOURS SHIFT DIFFERENTIAL $5,000 FT POSITION SIGN-ON BONUS $2,500 PT POSITION SIGN-ON BONUS Position Overview: EverHeart Hospice is currently seeking a compassionate and dedicated Registered Nurse (RN) to join our team for after-hours coverage at our Hospice Care Center in Greenville, OH. This vital role involves providing direct patient care during evenings, nights, weekends, and holidays, ensuring comfort, dignity, and support for both patients and their families throughout every stage of care. Responsibilities: Responsibilities of Position: Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including: • Inpatient Care Center. • SIA/Continuous Care. • Referrals and Admissions. • Chart Reviews. • Other duties as assigned. • Provide patient/family services according to established Plan of Care and signed physician orders. Consult the patient’s physician and/or Medical Director when necessary and initiate appropriate telephone orders for changes in the Plan of Care. • Coordinate IDG planning for assigned patients. • Participate in the IDG analysis of the patient, resulting in the development, implementation, and revision of the IDG Plan of Care. • Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers. • Facilitate continuity of care by coordinating the transfer/admission of home-care patients to an inpatient setting. • Facilitate continuity of care by coordinating the transfer/admission of inpatients to home-care settings, if applicable. • Explain hospice services and benefits regarding Medicare, Medicaid, and Private insurance, and know when to use Palliative Care Interventions and ABNs. • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate. • Order appropriate Durable Medical Equipment (DME) for the patient. • Perform competent initial nursing assessments. • Work evenings and weekends as needed to meet the needs of the patients and families and the agency's backup schedule. • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements. • Maintain clinical competency for assigned responsibilities and procedures. • Other duties as assigned. Qualifications: Education and Experience: • Graduation from an approved School of Nursing. • Current RN licensure in the State of Ohio. • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing. • Hospice experience preferred. Special Skills and Requirements: • Shall have a valid driver’s license. • Excellent customer service skills. • Detail-oriented with excellent communication, documentation, and organizational skills. • Ability to follow instructions. • Ability to work independently without supervision. • Travel to other sites as needed. Travel may be in inclement weather. • Ability to lift 25 lbs of supplies or equipment. • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking. • Ability to assist with patient care activities such as ambulation, transfers, and positioning.
