Position Summary:
The Care Coordination Specialist is responsible for supporting members through comprehensive care planning, intake, and Enhanced Care Management (ECM) services. This role works collaboratively with internal teams and external partners to ensure timely, coordinated, and person-centered care.
Key Responsibilities:
Complete comprehensive intakes and assessments for new and existing members
Develop, implement, and maintain individualized care plans in collaboration with members and care teams
Provide Enhanced Care Management (ECM) support in alignment with program requirements
Coordinate services across medical, behavioral health, and community-based providers
Conduct outreach and follow-up with members to ensure engagement and continuity of care
Document all activities accurately and timely in the electronic health record (EHR)
Support service close-outs to ensure services are billable and compliant
Participate in weekly ECM and care coordination meetings
Maintain compliance with all regulatory, contractual, and organizational standards
Collaborate with Care Coordinators, CRCs, leadership, and external partners as needed
Qualifications:
Bachelor’s degree required (healthcare, social work, public health, or related field preferred)
Minimum of 1-2 years of experience in care coordination, case management, or a related healthcare role
Knowledge of ECM, care management models, and healthcare systems preferred
Strong communication, organizational, and time management skills
Ability to manage multiple cases and meet deadlines
Proficiency with EHR systems and Microsoft Office/Google Suite
Bilingual skills a plus (if applicable)

Compensation & Benefits
Salary: 48,000-58,000
