About the Role:

As a Care Coordinator at Assisting Hands Home Care North & South Charlotte, you will play a vital role in enhancing the lives of our clients through personalized care management. The right candidate must enjoy working with seniors and adults needing home care.

Responsibilities:

Coordinate and oversee client care plans to ensure quality service delivery.

Conduct assessments and evaluations to determine client needs and preferences.

Communicate effectively with clients, families, and caregivers regarding care updates.

Schedule and manage caregiver assignments to ensure optimal client support.

Monitor client progress, adjust care plans as necessary and report changes to our RN.

Provide training and support to caregivers to enhance service quality.

Maintain accurate and up-to-date client records and documentation.

Assist with marketing and community outreach initiatives to promote services.

Requirements:

Previous experience in healthcare or home care coordination preferred.

Strong communication and interpersonal skills to foster client relationships.

Ability to multitask and prioritize in a fast-paced environment.

Proficient in using electronic health records and scheduling software.

Compassionate demeanor and a commitment to client-centered care.

Valid driver's license and reliable transportation required.

Willingness to work flexible hours to meet client needs.

Active NC CNA license.
