    Job Summary

    We’re looking for a dedicated Care Coordinator to join our growing team. In this role, you’ll be the bridge between client, families, and care providers, ensuring every client receives high-quality, personalized non medical care. You’ll help design care plans, oversee client goals and support caregivers, making a meaningful difference in people’s lives every day.

    The ideal candidate has over 2 years of hands-on caregiving experience, is compassionate, organized, and enjoys helping others thrive in their care journey.

    Key Responsibilities

    Collaborate with clients, families, and care staff to deliver seamless care

    Coordinate and oversee care plans tailored to individual needs

    Develop and monitor personalized care plans and goals

    Educate clients on their care options with clarity and empathy

    Recruit, train, and support staff to uphold excellent standards of care

    Create and manage caregiver schedules to ensure reliable coverage

    Caregiving when the need arises

    Qualifications

    Previous experience in caregiving and Care Coordinator or in a similar role preferred

    Certification as a MA, CNA, LPN is a plus

    Strong organizational and problem-solving skills

    Ability to manage multiple priorities in fast-paced situations

    Compassionate communicator with excellent interpersonal skills

    Strong verbal and written communication abilities

    Must have a valid drivers license and have reliable transportation
