    Job Summary

    We are seeking a Care Coordinator to join our team. In this role, you will be responsible for managing the intake process for all new clients, developing and implementing individualized service plans (ISPs), and ensuring coordinated, high-quality care throughout the client’s service journey. This role acts as a bridge between clients, families, caregivers, and the CCHC team to promote a seamless onboarding experience, maintain regulatory compliance, and support overall client satisfaction.

    Responsibilities

    Manage the entire intake process, including receiving referrals, verifying eligibility, and collecting necessary documentation.

    Enter and maintain accurate client demographic and insurance information in HHAexchange.

    Conduct initial comprehensive needs assessments in alignment with ODP/OLTL regulations.

    Collaborate with clients, families, and the care team to develop individualized service plans.

    Serve as the primary point of contact for clients, families, caregivers, and service providers.

    Coordinate multidisciplinary care to address all client needs.

    Monitor client progress through in-home visits, phone calls, and care team meetings.

    Track authorized services and ensure compliance with EVV requirements.

    Conduct quarterly audits of client files to verify accuracy and compliance.

    Collaborate with scheduling and staffing teams to align client needs with caregiver availability.

    Monitor ongoing authorizations and notify appropriate staff of expirations or changes.

    Maintain accurate and complete client records in HHAexchange and agency databases.

    Support the billing process by verifying authorized service hours against care delivered.

    Actively identify and follow up on new client leads and referrals.

    Participate in community outreach efforts to promote CCHC’s mission and services.

    Qualifications

    High school diploma or equivalent required.

    Associate’s or Bachelor’s degree in Human Services, Social Work, Nursing, or related field preferred.

    Minimum 2–3 years of experience in client intake, service coordination, or case management within home care, developmental disability services, or a related field.

    Prior experience with HHAexchange and EVV compliance is strongly preferred.

    Excellent interpersonal skills and high level of compassion.

    Strong verbal and written communication skills.

    Occasional lifting of files and supplies (up to 25 lbs).

    Occasional travel to client homes, community meetings, and events.
