Job Summary

We are seeking a Chronic Care Manager to join our team. In this role, you will work collaboratively with patients to determine their medical needs and goals, develop the best course of action, and oversee their treatment plans, ensuring each patient gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices.

Responsibilities

Collaborate with physicians, patients, families, and healthcare staff

Coordinate a variety of healthcare programs

Develop individualized care plans

Educate patients on their healthcare options

Create goals and monitor progress toward goals

Regularly reconcile medications

Improve health to reduce ER utilization and hospital readmissions

Connect patients and their families with relevant resources

Serve as the contact point, advocate, and informational resource for patients and their families

Qualifications

Previous experience as a Chronic Care Manager or similar position is required

At least 3 years of experience as a Medical Assistant is required

Certification as a medical assistant is preferred

Experience in primary care is preferred

Past leadership experience is a plus

Strong problem-solving and organizational skills

Ability to manage multiple projects or tasks and prioritize appropriately

Ability to work in fast-paced situations and make sound decisions quickly

Excellent interpersonal skills and high level of compassion

Strong verbal and written communication skills

Experience with Athena EHR is a plus
