    Job Summary

    The Care Coordinator is responsible for coordinating, implementing, and supervising the delivery of services to agency clients in alignment with the company’s mission and standards of care. This position is accountable for adherence to regulatory and agency requirements for service provision and ensures compliance with and maintenance of client records. Additionally, the Care Coordinator will support business growth by visiting and building relationships with facilities, hospice organisations, senior living communities, rehabilitation centres, hospitals, discharge planners, and other referral partners in and around Myrtle Beach.

    Responsibilities

    Introduce the agency's non-medical home care services and pursue referral opportunities while documenting all outreach activities and reporting outcomes to management.

    Collaborate with physicians, patients, families, and healthcare staff

    Coordinate a variety of healthcare programs

    Develop individualized care plans

    Educate patients on their healthcare options

    Create goals and monitor progress toward goals

    Create schedules

    Overseeing Caregivers

    Qualifications

    Minimum qualification: Certified Nursing Assistant (CNA) required

    Minimum 2 years of home care field experience required

    Experience in care coordination, supervision, case management, or client services preferred

    Experience conducting client assessments and supervisory visits

    Experience supporting caregivers and monitoring service quality

    Experience with scheduling, documentation, and compliance

    Strong communication, customer service, and relationship-building skills

    Ability to represent the agency professionally and promote services to referral sources

    Strong organizational and time management skills

    Ability to work independently and as part of a team

    Valid driver’s license

    Willingness to travel within the service area

    Basic computer skills and the ability to complete documentation accurately
