Under direct supervision, the Care Coordinator is responsible for overseeing and supporting the delivery of home care services to the patients/clients of Dignified Hope Care Inc. (DHC). Home care services include companionship, light housekeeping, meal preparation, monitor and recording vital signs and wellness information, medication reminders, and other non-medical support within the scope of the Home Care Aide certification (HCA) and the scope of that Care Coordinator’s additional certification(s) or license(s) which may include CNA, EMT, or MA certification.
Additionally, the Care Coordinator works alongside the Executive Care Coordinator and/or other Care Leads to develop and maintain staff schedules, provide training and mentorship to care staff, and collaborate to develop, update, and coordinate patients' individualized care plans to ensure continuity and high-quality care delivery.
