Position Overview
We are seeking a Licensed Practical Nurse (LPN) to join our leadership team as a Care Coordinator. This is a field-based role focused on client assessments, caregiver supervision, and ensuring exceptional care from day one.
This position is ideal for an LPN who wants more autonomy, variety, and purpose than a traditional clinical setting.
What You’ll Do
Conduct in-home assessments for new clients and create care plans
Perform supervisory and follow-up visits with existing clients
Supervise, coach, and support our caregiver team
Lead caregiver introductions for new clients (30–60 minutes per introduction)
Translate care plans into clear, actionable guidance for caregivers
Use WellSky to review schedules and coordinate care
Work with leadership and our RN to resolve client concerns
Participate in weekly leadership team meetings
Occasionally step in to cover caregiver shifts if needed
What We’re Looking For
