    Job Title: Care Coordinator Location: Fargo, ND Job Type: Full-Time Schedule: Monday–Friday, 8:00 AM – 5:00 PM (with occasional on-call responsibilities)

    Position Summary:

    The Care Coordinator plays a vital role in ensuring our clients receive the highest standard of care. This position involves scheduling caregivers, maintaining client and caregiver relationships, managing care plans, and coordinating services in a fast-paced environment. The ideal candidate is organized, empathetic, and able to balance administrative tasks with compassionate communication.

    Key Responsibilities:

    Coordinate and maintain caregiver schedules in alignment with client needs and preferences.

    Serve as the main point of contact for clients, families, and caregivers.

    Monitor and adjust care plans as needed, ensuring continuity and quality of service.

    Participate in on-call rotation to support clients and staff after hours.

    Document all communications, schedule changes, and care updates accurately in the system.

    Assist with caregiver hiring, onboarding, and training coordination as needed.

    Maintain compliance with company policies, licensing standards, and state regulations.

    Qualifications:

    High school diploma or equivalent required

    Proficiency in Microsoft Office and care scheduling software (ClearCare/WellSky preferred).

    Excellent communication and interpersonal skills.

    Strong organizational skills and ability to multitask.

    Compassionate, patient, and client-focused demeanor.
