Key Responsibilities
Assessment & Care Coordination
Complete non-clinical client assessments (psychosocial, functional, environmental needs)
Assist with development and updates of individualized care plans in collaboration with clinical staff
Coordinate services and referrals with nurses, providers, and community resources
Monitor service delivery and follow up on unmet client needs
Documentation
Maintain accurate and timely documentation of assessments, care coordination activities, and client progress
Ensure records meet agency, funding, and Georgia state requirements
Compliance & Regulatory
Follow Georgia regulations related to home and community-based services
Maintain HIPAA compliance and client confidentiality
Participate in audits, quality improvement activities, and corrective actions as needed
General Duties
Communicate regularly with clients, families, and service providers
Participate in team meetings and case reviews
Perform other duties as assigned to support agency operations
Performance Expectations
Complete assessments within required timeframes
Maintain accurate, timely documentation with minimal errors
Ensure assigned clients receive coordinated services without service gaps
Meet caseload productivity standards established by the agency
Demonstrate professional communication and timely follow-up with clients and providers
Maintain compliance with agency policies and state regulations
Minimum Requirements
Bachelor’s degree in Sociology, Psychology, human services, social work, healthcare, or related field preferred
Minimum 1 year experience in care coordination, case management, home health, or community-based services preferred
Knowledge of community resources and social service systems
Strong organizational, documentation, and communication skills
Valid Georgia driver’s license and reliable transportation
Ability to pass background check, TB screening, and drug screening per Georgia requirements
Ability to work independently as a contractor
