CARE COORDINATORS work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes. They effectively educate and link patients and providers to available community-based resources. Following are primary areas of responsibility

MINIMUM QUALIFICATIONS

Education: High school diploma or equivalent
Skills and Abilities:
Minimum two years’ experience working with patients and the public
Knowledge of federal and state regulations applicable to a clinic
Must possess successful problem-solving and decision-making skills
Ability to prepare and meet timelines prior to deadlines
Ability to write reports, business correspondence and care plans
Ability to effectively present information and respond to questions from groups of managers, clients, patients, regulatory authorities, and the public
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form
Ability to multi-task and problem-solve
Interpersonal Communications: Strong oral and written communication skills; represent SMCHC positively to patients and the public; follow written and oral instructions. Confidentiality: Keep all patient and personnel information confidential and within the confines of any authorization for release of information agreements.
PREFERRED QUALIFICATIONS · Valid Missouri LPN, social worker or other related licensure or training · One year of experience in community health, human services, outreach and/or healthcare · Previous case management experience · CHW certification or ability to complete and achieve CHW certification · Medical Assisting certification or ability to complete and achieve MA certification
