Purpose: To provide personal care and assistance in monitoring the wellbeing and the environment of the client receiving assistance in their own home
Eligibility Requirements:
Ability to read and take blood pressure, pulse, temperature.
Must enjoy working with individuals with disabilities
Ability to establish effective relationships with client, family members and team members.
Must be able to read, comprehend, write and speak English and Spanish to communicate effectively with client, family, visitors and other members.
Must be able to perform duties and responsibilities with or without reasonable accommodation.
Essential Functions:
Review and understand clients services and medical plans
Assist with activities of daily living including bathing, dressing, grooming, toileting, eating, transferring, and ambulation as specified in clients medical plans.
Engages with client in meaningful conversation and socialization while providing care assistance.
Observes residents for changes in physical, emotional, mental, and social condition, unusual symptoms, accidents, injuries, or unusual symptoms, accidents, injuries, or unusual occurrences, and report to family
Assist with mealtimes, serves meals in accordance.
Understand clients diagnosis and medications and refills
Assist with requesting medical referrals and follow through until the referral is obtained.
Housekeeping:
Makes residents beds, empties trash and tidies client home daily and as needed or requested.
Changes resident linens and towels according laundry schedule, and as needed or as requested.
Maintains clients clothing and closet in a neat and orderly manner.
Assist with clients laundry
Assist in disinfecting walkers, wheelchairs, shower chair, and other adaptive equipment
Assist with other housekeeping duties as assigned.
Other:
Assist with scheduling medical transportation for client
Assist with attending medical appointments with client and riding in the medical transportation
