    Executive Home Care of Augusta is a veteran-operated home care agency delivering concierge-level service to families across the CSRA. We're looking for a Care Manager who shares that conviction and wants to make a real difference in the lives of our clients and the caregivers who serve them.

    The Role

    As Care Manager, you are the heart of the operation — the bridge between our clients, their families, and our caregiving team. You'll oversee the quality and continuity of care across our client base while mentoring, supporting, and supervising the caregivers who make our mission possible.

    What You'll Do

    Conduct in-home client assessments and build personalized care plans that reflect each client's needs, routines, and preferences

    Supervise, coach, and support our caregiving team, ensuring every client receives consistent, concierge-level care

    Match caregivers to clients thoughtfully, with attention to skills, personality, and continuity

    Perform regular quality-assurance and supervisory visits in clients' homes

    Serve as the primary point of contact for families, communicating with warmth, clarity, and responsiveness

    Monitor changes in client condition and adjust care plans in coordination with families and healthcare providers

    Support caregiver onboarding, ongoing training, and compliance documentation

    Help maintain scheduling continuity and assist with on-call coordination as needed

    What We're Looking For

    Prior experience in home care, healthcare, case management, or a related field

    Strong leadership and people skills — you bring out the best in a team

    Genuine compassion for seniors and individuals needing care

    Excellent communication and organizational abilities

    Comfort using care management software and standard office tools

    A valid driver's license, reliable transportation, and willingness to travel within the CSRA
